Your Role With Us:
The MSW Care Manager works with patients, families, physicians and community providers to evaluate resource/referral needs and coordinate discharge planning for our patients. They complete a comprehensive Psychosocial Assessment for each assigned patient, administers MMSE’s per doctor’s requests, and completes an evaluation for all PMC patients. MSW Care Managers provide supportive counseling to patients and their families in consultation with the treatment team.
Completes a comprehensive Psychosocial Assessment
Completes MMSE’s per doctor’s request in a timely manner
Documents social work interventions and patient contacts
Performs social work activities including liaison with community providers to ensure timely discharge
Master’s Degree in Social Work from a CSWE accredited school required.
LMSW, LCSW or CSWA (pursuing LCSW) required by State of Oregon.
Required to complete Crisis Prevention Intervention training within the first 30 days after hire and annual recertification thereafter
