PacificSource Health Plans Traditional Health Worker Liaison in Hood River, Oregon

Overview

The focus of this position is to improve Traditional Health Worker (THW) access for Members and increase recruitment and retention of THWs within the community. The THW Liaison will act as a hub for Members and THWs in our service area. This position will collaborate with providers and community stakeholders to improve regional availability of THW services and to support service system improvements. Work includes collaborating with the OHA THW Commission and Office of Equity and Inclusion to implement best practices and meet regulatory requirements. This position will implement PacificSource’s THW Integration and Utilization Plan and will be responsible for data collection and submission to the State. The THW Liaison will work collaboratively with other departments involved in the work to coordinate and share progress. This role will conduct internal and external trainings to support the THW workforce.

Responsibilities

  • Implement PacificSource’s THW Integration and Utilization Plan in collaboration with key stakeholders. Ensure compliance with OHA regulations and the OHA’s Traditional Health Worker Commission’s best practices, under the Office of Equity and Inclusion.

  • Integrate THWs into the delivery of services. Outreach to current and future employers of THWs. Share training and certification processes and payment models. Work collaboratively with Provider Services to onboard new providers.

  • Collaborate with community partners to build shared annual work plans, deploy trainings and facilitate community coordination that is responsive to stakeholder needs. Maximize opportunities for shared learning.

  • Develop THW communication plans to members, providers and community based organizations (CBOs). Share the benefits of and access to THW services.

  • Develop work plans to increase THW utilization. Oversee the work and report progress. Document location and type of all THWs and update regularly. Settings to include clinic, non-clinic and CBOs. Identify gaps and mitigation plans.

  • Work closely with the analytics department to develop baseline utilization measures and performance indicators over time.

  • Work in collaborationwith case management Personal Health Navigators to understand regional needs of our membership.

  • Align work with PacificSource strategic goals and major initiatives, the Health Council community governance model and the CHA and CHIP. Engage the Clinical Advisory Panel and the Community Advisory Council in the THW work.

  • Identify barriers that inhibit the ability to meet goals, develop mitigation plans and oversee interventions.

  • Create detailed reports of project progress as required by contracts and regulations. Ensure timely submission to the OHA.

  • Develop member satisfaction survey with THW services, aggregate results and develop improvement activities.

  • Serve as a resource for internal staff, community partners and provider partners on all THW activities

  • Represent PacificSource in the communities and regions we serve to build collaborative partnerships. Attend local and state meetings and participate in pertinent workgroups.

Supporting Responsibilities:

  • Meet department and company performance and attendance expectations.

  • Participate in and support project teams led by other departments and provide necessary input to support the goals of colleagues

  • Follow the PacificSource privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.

  • Perform other duties as assigned.

Qualifications

Work Experience: Two years of experience in healthcare, public health or community based organization.Prefer experience as a THW or in health care coordination services. Experience preferred in community networking and/or facilitating groups.

Education, Certificates, Licenses:High school diploma. THW training and certification preferred. Associates or Bachelor’s degree preferred.

Knowledge: Knowledge and understanding of social determinants of health and health equity and a commitment to diversity and inclusion Program implementation to reach desired outcomes. Relationship building and community collaboration. Experience giving presentations. Organizational skills with solid experience in using computers and various software applications. Ability to work independently with minimal supervision. Bi-lingual strongly preferred.

Environment:Work inside in a general office setting with ergonomically configured equipment.

Travel is required approximately 50% of the time. Rare overnight travel.

Physical Requirements:Requires sitting, standing, walking and stooping/bending according to work tasks. Light lifting and carrying of files and department notebooks. Must be able to read text and numbers and communicate clearly and distinctly using a voice telephone and in person. Handles files and use of computer keyboard and writing instruments.
