This Client Wellness Facilitator position will work at LifeWorks NW's Certified Community Behavioral Healthcare Clinics (CCBHC) site in Gresham, OR.

In 2017, LifeWorks NW was selected to represent Oregon in the federal demonstration project to implement CCBHCs to advance delivery of behavioral health care. LifeWorks NW's CCBHC sites provide comprehensive client care through a team-based approach with services that include mental health and substance use counseling, peer-delivered health and wellness services, primary care services and psychiatric rehabilitation. Staff that work at our CCBHC sites collaborate with others to care for individuals through daily interactions and support, counseling, skills training, care coordination, health promotion and education, medication management services and supportive services with preventive approach.

Purpose:The Client Wellness Facilitator assures that clients of CCBHC receive screening and follow up on essential health metrics. They will serve as the liaison between the CCBHC site and other health care systems, including primary care services (both internal and external primary care). They will provide care coordination and case management within the CCBHC and with outside medical providers as it pertains to the client's health and wellness goals so as decrease ER usage, hospitalizations and improve overall client health.

Days/Hours:Monday through Friday, 9:30 am to 6:00 pm

Pay/Benefits:$18.81 to $20.45 per hour.Full benefits.

Essential Responsibilities:

  • Completes health screenings (i.e. tobacco use, BMI, blood pressure, diabetes, etc) for CCBHC clients.

  • Requests and gathers labs from internal and external primary care providers.

  • Assures that required labs are completed on an ongoing basis for CCBHC clients.

  • Assures that CCBHC health metrics are gathered and documented on all CCBHC clients.

  • Provides as needed back up and support to the RN such as gathering data, connecting clients to resources.

  • Runs reports regarding health metrics and facilitates outreach and support from the various CCBHC teams to better support the client.

  • Has an awareness of internal and external health resources and facilitates connecting clients to those resources.

  • Coordinate with internal and external teams to help in medication adherence.

  • Facilitates coordinate between various medical services both inside and outside the CCBHC, including, but not limited to: internal health home primary care, internal psychiatric services, external primary care, external specialty health care.

  • Identifies primary care physicians or assists clients in establishing a primary care provider.

  • Provides and coordinates wellness activities for CCBHC clients (e.g. health education, smoking cessation).

  • Enters all required data into agency specific electronic health records system and program specific tracking systems, provides regular reports as requested.

  • Continually increase level of cultural sensitivity, awareness and competency.

  • Follows up with physicians to ensure communication is achieved within specified time frames.

  • Checks email/voicemail daily/throughout the work day, and when working, responds to contacts within 24 to 48 hours.

  • Provides back up and support to co-workers in providing assistance during medical or mental health emergencies.


  • Bachelor's degree in behavioral sciences field and minimum three years relevant work, education, training, or experience (QMHA qualifications) required.

  • Experience in a health setting preferred, particularly a primary care health setting

  • Understanding of basic medical terminology and health conditions.

  • Good computer skills including a functional knowledge of Microsoft Outlook and Word. Ability to master proprietary software including electronic health record system.

  • Demonstrates effective communication skills and maintains a cooperative working relationship with others in a culturally diverse environment.
